Short Term Rehabilitation Services

In consultation with primary care physician, nursing staff, and team of skilled health care providers, our highly trained therapists develop individualized long term rehabilitation services for our long-term residents as well as for short term rehabilitation therapy. Our rehabilitation services which includes physical, occupational, & speech therapy provides services 6 days a week.

Success in rehabilitation relies on persistence and commitment. Our experienced rehabilitation team works with patients to restore mobility and improve their quality of life. We provide rehabilitation programs to serve patients with a variety of physical ailments.

Our long-term residential health care program is designed to address the needs of patient populations who require different levels of care, whether they function with a greater degree of independence, need assistance with activities of daily living; or require medical attention for conditions that are clinically complex. Medical Model Adult Day Care Center For Bronx, NY

Bay Park’s adult day care center is an alternative to nursing home placement, where the frail, elderly or disabled young adults can spend time away from home in a supportive environment. Our program also gives participants the opportunity to meet and establish supportive relationships with others who experiencing similar life situations, medical conditions & psycho-social needs.

Our adult day care center provides services 7 days a week to maintain participant’s well-being in the community.

Respite Services

Respite services allows individuals with special needs to enter the facility for a temporary stay (from several days to three weeks) while their caregivers are away or taking temporary relief from the emotional and physical stresses which are often associated with caregiving. We know caregiving can be a stressful task, which is why we are happy to provide respite care!  We hope you take advantage of our respite care services and give yourself or a caregiver you know a nice break!

All Respite guests reside in a beautifully appointed private room.  During respite stay, each guest will receive individualized care and attention and a personalized care plan which addresses each guest specific care needs.
